Job Overview Shriners Children's Ohio has an opportunity for a part-time

Responsibilities The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.
Qualifications Education BSN required, MSN preferred
Certification/Licensure Ohio RN License
Case Management Certification (CMC or ACM) is required or obtained within 2 years of hire or eligibility
BLS is required or obtained within 30 days of hire
Experience Required:
3 years RN experience
Conducting and documenting comprehensive assessments of patient's health status and psychosocial needs, including health literacy status and deficits, and ability to develop a collaborative plan with the patient and caregiver.
Disease management and application of motivational interview techniques experience
Preferred:
1+ year of pediatric case management experience
Utilization review/management experience with a working knowledge of InterQual criteria is strong preferred.
Experience with funding sources, healthcare delivery services, clinical standards and outcomes management.
Demonstrated ability to mitigate barriers to patient/family adherence, facilitate care transitions, and improve health outcomes in a pediatric setting.
5+ years of clinical experience in burn care and cleft lip and palate conditions.
Bilingual in English and one additional language
