Job description
Seeking an APN with inpatient experience, home care exp preferred. Hospital at home APN is a hybrid role. APN will be instrumental in identifying and screening potential referrals and completing their H&Ps. Home visits if clinically needed.
The Hospital at Home Advanced Practice Nurse Provider (APN) will serve in a dual capacity. The APN will take a lead role in identification and assessment of potential Hospital at Home referrals, in collaboration with the Virtual Hospitalist. The APN must be committed to providing high-quality, evidence based, patient centered care to a diverse patient population in a non-traditional setting. Additionally, the APN will conduct in-home patient visits to Hospital at Home patients. This is not a predominantly patient home visit position. This position serves to identify patients that can be treated in this new Hospital at Home program.
The Advanced Practice Nurse II performs the role utilizing the patient-centered coordinated care model demonstrated through the following competencies: leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making.
Needs strong communication skills, ability to network, and compassionate bedside manner.
