Short Description

Under the leadership of the AVP Care Management and Healthcare Access, the Senior Director Care Management will be responsible for the strategic direction setting, management, continuous improvement, and maintenance of the Clinical/Utilization Review and Transitional Case Management Team processes. They are a subject matter expert in all aspects of case management, providing direct onsite support, ensuring best practices, and change management. This role evaluates the efficiency, appropriateness, necessity of the use of medical services, procedures, and level of care for patients. Uses data analysis, and develops and implements strategies to improve workflows and processes/procedures to enhance utilization review and discharge planning. Works to ensure care is delivered to patients at Cooper in appropriate time frames to maximize patient flow and reimbursement.

Ensures that discharge planning is thorough, compliant with policies and regulatory requirements, and patient centered. Must possess excellent communication skills, strategic and innovative visions, and proven motivational and change management capabilities. Must have an executive presence and a demonstrated record of accomplishment for delivering results and attaining goals. Collaborates closely with both internal and external partners. Responsible for improvement of department Key Performance Indicators. Implements and develops departmental policies and procedures. Allocates resources efficiently including staffing, budgets, and technology. Supports the organizational goals, mission and vision of Cooper.

Experience Required

Minimum of 5 years prior case management experience required

Education Requirements

MASTER'S degree required
