Job description
Job Overview:
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our hospice care team. As a Hospice CNA, you will play a crucial role in delivering high-quality, personalized end-of-life care to patients in their homes or hospice facilities, ensuring their comfort, dignity, and overall well-being during this sensitive phase of life.
Responsibilities:
* Patient Care:
- Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and monitoring vital signs.
- Document any changes in the patient's condition and report concerns to the registered nurse or hospice team.
* Emotional Support:
- Offer compassionate companionship and emotional support, fostering an environment of empathy and understanding for patients and their families.
- Listen actively, ensuring the psychological and emotional needs of the patients are met.
* Medication Administration:
- Assist in administering medications as per prescribed treatment plans and report any adverse reactions or side effects.
- Maintain accurate records of medication administration.
* Comfort Care:
- Maintain a clean, safe, and comfortable environment, making necessary adjustments to ensure patient well-being.
- Implement appropriate pain management techniques as outlined in the patient's care plan.
* Documentation:
- Maintain detailed patient records, documenting care provided, observations, and any changes in the patient's condition.
- Ensure compliance with all documentation and reporting requirements as per hospice regulations.
* Collaboration:
- Work collaboratively with the interdisciplinary team, including nurses, social workers, chaplains, and volunteers, to deliver holistic care to patients.
- Participate actively in team meetings and contribute to care planning discussions.
* End-of-Life Care:
- Support patients and their families in the transition to end-of-life care, providing comfort and assistance as needed.
- Offer respite care to family members to alleviate caregiver stress when appropriate.
* Education and Advocacy:
- Educate patients and families on the hospice philosophy and available resources.
- Advocate for patient rights and preferences, ensuring their wishes are respected and fulfilled.
Role Requirements:
- Certification: Valid Certified Nursing Assistant (CNA) certification.
- Compassion: Demonstrated empathy, sensitivity, and compassion for hospice patients and their families.
- Communication Skills: Strong verbal and written communication skills for effective interaction with patients, families, and the hospice team.
- Attention to Detail: Exceptional organizational skills and attention to detail for accurate documentation and patient care.
- Team Player: Ability to collaborate within a multidisciplinary hospice team for comprehensive patient care.
- Physical Stamina: Capability to assist patients with mobility, transferring, and personal care while being on feet for extended periods.
- Ethical Conduct: High ethical standards, maintaining patient confidentiality and respecting autonomy.
- Reliability: Punctuality and dependability for consistent patient care.
- Valid Driver's License: Some roles may require a valid driver's license and reliable transportation for home visits.
