Boulder, CO
Compensation: $80,000 - $85,000 in salary, sign-on bonus, benefits, and UNLIMITED PTO!
Our client is looking for Registered Nurses to join their growing home health care team in the Longmont/Boulder, CO area. WE ARE OPEN TO RELOCATING QUALIFIED CANDIDATES THAT HOLD A COMPACT LICENSE

Our client is a venture-backed start-up, currently in stealth mode, and well-funded by world-class investors and phenomenal digital health leaders. The company's founder grew up on Medicaid (public insurance) and personally knows the desperation of being unable to afford necessary medical care.
The Role:
*****This role is 70% in person, 30% remote*****
Our client is looking for a reliable and passionate registered nurse to join their home health care team in the Boulder, CO area (Boulder/Lafayette) as an RN Case Manager. WE ARE OPEN TO RELOCATING QUALIFIED CANDIDATES THAT HOLD A COMPACT LICENSE. You will help develop a plan of care and deliver high-quality, skilled nursing for the families of children with intellectual and developmental disabilities (IDD).
In this Role you will:
- Visit the homes of patients every 60 days.
- Create care plans, provide care in-home to children, and oversee patient progress.
- Inform parents about their children’s health and provide coaching support.
- Leverage telehealth to scale the impact of your time, communicate with the care team, and interact with families.
- Be the critical voice for working to help children meet great health outcomes and furthering our client's mission.
Requirements:
- Must be a registered nurse with at least 2 years experience and currently licensed in the state of Colorado or hold a compact license
- Experience working with and training family CNAs, preferred
- Must live in the Boulder, CO area or be willing to relocate
- Must have CPR certification.
- Home care experience REQUIRED
- Pediatric experience, preferred.
- BLS certified, required
- PALS certified, preferred

*****Daily Schedule*****:
Monday:
- Mandatory Nurse meeting in office in Denver 9am-10:30am
- The rest of the day is admin day for nurses to work for home and get all their tasks completed
Tuesday-Friday:
- In-person start of care assessment - Max 3 per week. Usually only 1 per day
- The rest of the time is working remotely to chart, connect with families, and get case management work completed
