Job description
The Population Health Acute Care Coordinator responsible to perform key case management functions to ensure continuity of care and services for a defined population. Utilizes the nursing process to transition patients from the acute care setting to the least restrictive next site of care with the goal of preventing crises that lead to readmissions. Ensures continuity of care by collaborating and communicating with physicians, nursing managers, case managers, post-acute case manager and interdisciplinary team members. Is accountable for the coordination of high quality, effective, and appropriate health care transitions. Is accountable for data capture and analysis, outcome achievement and fiscal responsibility by maximizing favorable utilization patterns and minimizing unfavorable expenditures. Education: Graduate from an accredited school of nursing. BSN preferred. Experience: Minimum of 3 years previous nursing experience. Varied clinical experience preferred.
