Job description

We are seeking a Dedicated Outpatient And Military Loving RN (Registered Nurse) to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

Responsibilities:


  • Administer nursing care to ill, injured, or disabled patients

  • Diagnose and establish patient treatment plans

  • Monitor and report changes in patient symptoms or behavior

  • Communicate with collaborating physicians or specialists regarding patient care

  • Educate patients about health maintenance and disease prevention

  • Facilitate referrals to other healthcare professionals and medical facilities

  • Maintain accurate patient medical records

  • Provide advice and emotional support to patients and their family members

Qualifications:


  • Previous experience in nursing or other medical fields

  • Familiarity with medical software and equipment

  • Ability to build rapport with patients

  • Strong problem solving and critical thinking skills

  • Ability to thrive in a fast-paced environment

  • A diploma or degree from a National Council of State Boards of Nursing (NCSBN) accredited school of Nursing.

  • A current unrestricted active license to practice as a Registered Nurse from one of the 50 states, District of Columbia, Puerto Rico, Guam, or the U. S. Virgin Islands.

  • Maintain current certification in Basic Cardiac Life Support (BLS) for the Health Care Provider by the American Heart Association (AHA), American Red Cross (ARC), American Safety and Health Institute (ASHI), or the American College of Emergency Physicians (ACEP).

  • A minimum of two (2) years of professional experience in the field in the past 3 years; with at least one year of ambulatory care experience in family practice or internal medicine in the past 5 years.

  • "No adverse information on file" with the National Practitioners Data Band (NPDB).

Duties:

A. Assesses patient’s needs for nursing care, plans for nursing interventions, effectiveness of the interventions and alters the nursing plan and interventions appropriately.

B. Documents all nursing care provided to include assessments and evaluations in accordance with applicable regulations and standards of practice.

C. Provides patient education, disease management and ongoing monitoring in coordination with the Primary Care Manager.

D. Provides care coordination care between varieties of partners involved in beneficiary care.

Communicates, updates, reports and coordinates with the HSWL-SC RN Field Technical Manager regarding all complex cases in accordance with Coast Guard policies and directives.

f. Provides nursing triage to acute issues in addition to regular scheduled appointments, with immediate evaluation and management of emergent problems as they occur.

g. Provides coordination of care in accordance with standards of practice established at the clinic for referral management and obtains treatment records for health record entry.

h. Identifies medical and dental readiness deficiencies through health record reviews and recommends established preventive services associated with physical exams and health surveillance requirements. (e.g. Periodic Health Assessment, Occupational Medical Surveillance and Evaluation Program).

i. Participates in the Health Services Quality Improvement Program, as directed by the Clinic Administrator.

j. Participates in attending and providing (ad hoc) training at the clinic for staff.

k. Administers oxygen, IV fluids, and prescribed medications (i.e. oral, nasal, optic, ophthalmic, rectal, intradermal, subcutaneous, intra-muscular, etc.) and immunizations in accordance with provider orders and regulations. Reports and documents adverse reactions.

l. Collects specimens as directed for laboratory analysis and prepares for shipment as needed.

m. Prepares treatment room for patient encounters to include surgical and or diagnostic procedures.

n. Prepares, operates, and monitors specialized equipment such as cardiac monitors, defibrillators, IV pumps, etc.

o. Performs front desk operations as needed to support clinic operations; schedules medical and dental appointments, provides patients with intake forms and maintains patient reception area.

p. Inventories medicine, drug stocks and other consumables to ensure adequate supplies and controls as needed.
