ABOUT THE OPPORTUNITY/WORK The PRISM Case Manager, as part of the PRISM team, is a program staff providing direct service by providing comprehensive and coordinated case management for women enrolled in the PRISM program. The PRISM Case manager is responsible for executing a Plan of Care developed by the Case Manager and client with consultation from the PRISM therapist. The Case manager conducts outreach, ensure regular communication with referral sources as necessary, assesses the non-treatment needs of the client, links the client to ancillary services within the community, provides skill acquisition trainings, provides transportation to community-based appointments as needed, monitors the client's progress as it relates to substance use and reporting this to the therapist. This position reports to the PRISM Program Coordinator.
- Conduct comprehensive assessment of clients’ non-treatment needs, including but not limited to, focusing on specific needs of pregnant women. Develop Individual Plan of Care with the client in consultation with the PRISM therapist focused on client retention in treatment, completion of treatment, and continuing services to support sustained recovery. Coordinate services on the Individual Plan of Care by linking the client with community-based service organizations best suited to meet the needs. Provide a continuous assessment of needs, with input from the client and substance use therapist and amend the Individual Plan of Care accordingly.
- Consult regularly with community services and interagency programs to serve the client most effectively and make referrals as appropriate.
- Provide ongoing communication including regular face-to-face contacts with the PRISM therapist.
- Provide psychoeducation regarding pre-natal care, pre-natal exposure to substances and family planning, when appropriate.
- Participate as a member of the PRISM team and attend meetings as scheduled.
- Maintain records, manage and report assessment data following program guidelines and requirements.
- Provide and/or assist clients in arranging transportation for client to and from medical services or other community-based appointments as needed.
- Provide continuing care through active treatment and after completion of treatment.
- Supports/assists all members of the PRISM team with needs across the program.
- Other duties as assigned to support the program/agency as a whole.
Does This Describe YOU? - Bachelor’s degree in social work or a related field with appropriate State Licensure.
- Experience related to substance use treatment and case management.
- Selected candidates for this position must possess a sensitivity to the diversity of the Agency’s service population including (but not limited to) differences of culture, race, religion, gender, sexual orientation and socioeconomic characteristics.
- Demonstrated competence in providing gender specific case management as evidenced by related training, experience and/or education.
- Have or develop significant understanding of non-treatment systems, such as local OB/GYN practices, courts, corrections and Department of Health and Human Services.
- Special abilities and skills necessary to perform the required tasks and that best meet the needs of the Agency also will be considered. Any standard above may be waived when compensating specifications or circumstances exist.
