Job description
The University of Iowa Hospitals & Clinics is seeking a Discharge Planning Assistant to assist as an active member of the Care Coordination team, working collaboratively with the Nurse Navigators and Social Workers. The Discharge Planning Assistant is responsible for performing administrative and operational functions needed in organizing the Care Coordination Division. This position will provide direct administrative support to the discharge planning process. The Discharge Planning Assistant will work in a fast-paced, high-volume environment multitasking between telecommunication and web-based applications to deliver a high level of customer service in a timely manner. This position will work in a variety of clinical areas including the Inpatient units and Emergency Department and directly interact with patients and their families, clinical and ancillary hospital staff, and other internal and external customers.
Position Responsibilities
Percent of Time : 100%
Schedule : Monday – Friday, 7:30am-4:00pm

Required Qualifications
Bachelor’s Degree or an equivalent combination of education and experience is required.
A minimum of 6 months experience in health care.
Excellent written, verbal, and interpersonal communication skills.
Must be proficient in computer software applications.
Desirable Qualifications
Knowledge and understanding of medical terminology.
Knowledge of Post-Acute community resources.
Strong organizational and prioritizing skills
Experience in working with EPIC.
