MGB-9402 - Physical Therapist - Newton, MA - 13 Weeks Contract
Sigma Healthcare team is currently looking for a Physical Therapist to work onsite at our client's healthcare facility in Newton, MA.
Shift: 08:00 - 16:30
Job Summary: - Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes.
- Patients may range in age from birth to over 100 years.
- Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs.
- Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of our Home Care.
Requirements: - BLS/BCLS certifications
- Physical Therapist Professional license in Massachusetts 1 year Traveler and Home Care experience.
- 1 year Case Management Experience.
- Ability to travel to cases as assigned.
- Candidate Must be Vaccinated
