This is an in-office role.
The Donation Case Worker: provides onsite response, is responsible for advocating for donation and supports families through the organ and tissue donation process. Initiates timely donation conversation with families either in person or telephonically and in collaboration with hospital partners. Maintains authorization rates that are equivalent or exceeds the national average. Supports and fosters the Mission, Vision and Core Values of LiveOnNY and its commitment to organ and tissue donation.
- Evaluate and determine bereavement support needed for potential donor families throughout the entire designated service area.
- Responsible for educating the potential donor’s authorized person(s) about the opportunity for donation and requesting authorization.
- Obtain authorization or notification and conducts a Donor Risk Assessment Interview with appropriate identified individual(s).
- Collaborate with necessary parties to determine the best time to discuss the opportunity for donation with potential donor families after authorization trigger is met.
- Provide critical information related to family dynamics, identified support system and/or outcome of family conversations.
- Responsible for collecting basic information from the potential donor’s chart and providing organ referral information to Clinical Services and may serve as first responder.
- Refer donor families to LiveOnNY Aftercare Services Program, community agencies and/or provides appropriate referrals for counseling as requested.
- Serve as a family support resource and provides education and training for Clinical Services, Hospital Services, and external hospital and donation partners.
- BA/BS in counseling, psychology or social services, crisis intervention, or education OR 2 years of experience in organ procurement consent with proven track record is required.
- Excellent knowledge of the dynamics of grief and bereavement.
- Master’s in Social Work or related field preferred. Experience in trauma/critical care setting helpful.
Must possess a valid driver’s license and own a car. In person position.

Salary Range: $70,000-$80,000
