The Pharmacy Navigator Tech, Ambulatory role is a clinically based partnership with the outpatient pharmacy team that integrates with a nursing team on hospital inpatient units to identify which patients are being discharged and to assist in providing patients with medication at bedside. Responsibilities include clinically collaborating with patients, nursing, physicians, and case management to enroll patients in the pharmacy software, filling out all the required fields in the application, collecting all scripts, delivering the medications to the bedside, providing all the required documents to the bedside and obtaining payment information. The navigator will obtain patients allergies, health conditions, prescription insurance information, bill all required medications to the appropriate third party, and provide patient with a copayment summary. They are required to partner with necessary parties to assess/coordinate patient's financial responsibilities and obtain all necessary signatures as required by regulatory governing bodies. They are also required to work with various drug representative from different pharmaceutical companies to assist with helping patients with prescription affordability and any financial hardship that may occur. The navigator is responsible for working with the patient and educating patient on the appropriate counseling points and providing options for the patient at the bedside.
A day in the life of a Pharmacy Navigator Technician, Ambulatory at Hackensack Meridian Health includes:
1. The Navigator will be responsible for working with case managers, nursing staff, physicians, and unit clerks to identify which patients are being discharged.
2. The Navigator is responsible for completing all necessary and required fields in the pharmacy software or identified technology.
3. The Navigator is responsible for working with the patient and providing clinical education and options for the patient at the bedside.
4. The Navigator will be responsible for profiling new patient into the pharmacy information system, to include obtaining, receiving and/or providing all necessary information and documentation.
5. The Navigator will be responsible for collecting all scripts and delivering to the outpatient pharmacy.
6. The Navigator will be responsible for the delivery of medications to new patients.
7. The Navigator will also be responsible for delivering all required medication education and obtaining necessary signatures.
8. The Navigator will collect the payment and capture in the payment system.
9. The Navigator will work with pharmaceutical representatives to assist in getting patient prescriptions covered.
10. Prepares medication orders under the oversight of a registered pharmacist.
11. Assists the pharmacist in the maintenance of adequate drug supplies in filling areas and gives notification regarding the need to reorder supplies.
12. Assures that the filling areas maintained in a neat and orderly manner.
13. Refers requests for drug information to a registered pharmacist.
14. Assists the pharmacist in maintaining the security of medication and supplies.
15. Provides appropriate documentation wherever and whenever required.
16. Assists in maintaining patient medication profiles and records.
17. Assist with receiving, filling and selling of prescriptions.
18. Checks stock for expired, unusable and excessive items.
19. Performs tasks associated with automated equipment and maintains appropriate records as designated.
20. Prepare medication compounds according to 'recipe' once ingredients have been checked by the licensed pharmacist
21. Lifts a minimum of 10 lbs., pushes and pulls a minimum of 40 lbs. and stands a minimum of 4 hours a day.
Education, Knowledge, Skills and Abilities Required:
1. High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
2. Ambulatory experience with knowledge of medications.
3. Customer service experience.
Licenses and Certifications Required:
1. Valid New Jersey Pharmacy Technician Registration.
2. Valid Certification from PTCB or equivalent.
Working Conditions:
Lifts a minimum of 10 lbs., pushes and pulls a minimum of 40 lbs. and stands a minimum of 4 hours a day.
