Job description
The Referral and Admission Representative is responsible for the overall coordination of incoming referrals and initial visit scheduling under the direct supervision of the Team Leader - Admissions. The Referral and Admission Representative is a resource person for referral sources, and works closely with other members of the interdisciplinary team (IDT) to ensure adequate coordination and a smooth transition occurs to hospice services.
QUALIFICATIONS:
- A minimum of three years experience in customer service or admissions in a healthcare setting
- Billing/coding experience preferred but not required.
- Excellent communication skills.
- Ability to track and complete multiple tasks at one time.
- Ability to work independently; self-motivated.
- Actively supports and promotes philosophy of care and mission.
- Proficient at basic computer/keyboard skills.
RESPONSIBILITIES:
- Serves as the interface between the organization and referral sources to obtain and exchange referral information.
- Monitors ‘pending’ referrals and conducts necessary and appropriate follow up
- Coordinates and schedules assessment visits for Admissions Team staff
- Facilitates communication between the Directors of Clinical Care, the Director of Family Services, Admissions Team staff, Team Leaders, Care Delivery staff and other departments as necessary to assure the seamless delivery of care to new patients.
- Creates the initial referral record and any related orders in the electronic health record.
- Assists with coordinating pharmacy orders, medical supplies, infusion therapy and any durable medical equipment needed for new referrals and existing patients as requested by care delivery staff.
- In collaboration with Accounting staff verifies patient benefit information and obtains initial prior authorizations as necessary.
- Verifies physician license and related credentialing information on referrals by physicians new to the system.
- Effectively educates prospective patients, families, physicians and hospitals regarding the hospice benefit, organization services, and patient eligibility.
- Prepares reports as requested.
- Serves as a liaison between clinical and marketing staff.
- Identifies and reports areas for improvement and concerns for the safety of patients, family members and staff.
- Consistently conducts daily job responsibilities in a manner that maintains excellent customer service for patients, families, referral sources and staff.
- Performs other duties as assigned by the Team Leader - Admissions.
Schedule:
- 8 hour shift
- Day shift
- Monday to Friday with Holiday rotation.
Education:
- Associates Degree (Preferred)
- High school Diploma (Required)
