As an Elder Care Physical Therapist with Jordan Valley you will: - Work closely with the clinic to develop and maintain a treatment plan for elderly patients
- Perform patient assessment
- Establish plan of care
- Assess the effectiveness of treatment and modify treatment to achieve goals, providing feedback to the patient's primary care team on progress and areas of continued focus or need.
- Provide care following the patient's primary care team orders and established plan of care.
Requirements
- Bachelor's Degree, Master's Degree, or Doctorate in Physical Therapy from an accredited program
- Missouri license
- One year of experience is preferred; working with the elderly is a plus.
