Nurse Practitioner (Family Practice)
Fort Johnson (formerly Fort Polk) is an Army post located in Vernon Parish in Louisiana seven miles from Leesville, Louisiana. The city of Leesville, population 6700, is situated between Shreveport and Lake Charles. It is 63 miles to Lake Charles, Louisiana and 50 miles to Alexandria where the nearest municipal airport is located. Fort Johnson is the second largest employer in the state of Louisiana and has a huge economic impact on the entire state. The Army is situated on 198,000 acres. The Department of the Army actually owns 100,000 acres with the remaining acreage belonging to the United States Forest Service and part of the Kisatchie National Forest. The land that the post is on is both of historical and archeological value.

Employee provides independent and collaborative specialty, comprehensive and therapeutic health care services, integrating the art and science of nursing into medical practice. Medical management functions performed include assessment of acute, non-acute and chronic clinical and psychological diagnosis, new patient intake/screening, history and physical examination, laboratory and diagnostic studies; to include interpretation, ordering appropriate medications and treatments, providing staff education, and the initiation of a therapeutic plan in accordance with medical practice. Formats a therapeutic plan of care in collaboration with specialty physicians. Implementation includes major emphasis in the areas of patient/family education, assessment, comprehension, and support. The incumbent is responsible for the initiation, implementation and supervision of treatment plans and education for chronic conditions such as; but not limited to, coronary artery disease, endocrine diseases such as diabetes, hyperthyroidism, obesity, hyperlipidemia, and chronic kidney disease. Collaborates with physicians and other team members regarding patient management, to include evaluating and revising therapeutic plans.Provides educational management to patients and their families regarding disease processes and diagnostic tests; the therapeutic regimen including medication, diet and follow-up care; methods of adapting medication and treatment to current lifestyles. Provides counseling to patients and their families for prevention of illness and maintenance of health, in order to assist patients in attaining physical, mental, and social well-being. Oversees compliance with the preventive healthcare needs of adult patients visiting the cinic. Refers high-risk, unstable, or extremely complex patients to the physicians and emergency medicine. Performs emergency care as appropriate.Incumbent will be presenting patient cases and classes for patient care conferences in collaboration with other members of the health care team. Communicates patient management strategies to members of the patient care team. Collaborates with physicians regarding patient management, to include evaluating and revising therapeutic plans. Serves in a leadership position and is recognized as a consultant and role model to other health care professionals both within, and outside, the institution. Develops and/or conducts in-service education programs for other health care professionals. Serves as advisor/member of committees as needed. Develops, reviews, and appropriately updates policies, protocols, standards of nursing practice, standard operating procedures and scope of practice regarding adult, adolescent, pediatric and geriatric patient care in collaboration with the multidisciplinary members of the health care team.Performs other duties as assigned.
Must obtain and maintain a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.Incumbent will have to pass a pre-employment physical to meet Physical Efforts requirements. This position may require an annual physical or medical surveillance examination.This position requires the ability to lift 40 lbs or more.This is an inclement weather position. The incumbent is expected to make every attempt to report for duty on time and/or remain on duty during inclement weather conditions.Incumbent may be required to perform call duties; work overtime to include weekends and federal holidays, out of cycle, or unscheduled training or on an emergency basis. The work will require the incumbent to work evenings/nights; rotating shifts, extended hours or other schedules common to hospital operations.This position requires credentialing. Applicants tentatively selected for this position will be required to meet credentialing requirements.
