Job description
The Pharmacist Specialist is responsible for the provision of comprehensive clinical pharmacy services related to their area of practice through establishing and expanding clinical services and optimizing safe, effective, and cost-effective drug therapy. The specialist provides pharmaceutical care to achieve improved outcomes and quality of life for patients. Responsibilities include participation as member of the inter-disciplinary patient care team, provision of drug therapy in specialty area, patient and provider education, active participation on institutional committees, development of medication policies, protocols, order sets, medication utilization reviews, and QI projects to optimize drug therapy utilization. The clinical pharmacy specialist serves as a leader for the department in their specialty area through mentorship, education, and serving as a clinical resource for pharmacy staff, physicians, affiliate providers, nurses, and preceptor for pharmacy students on advanced clinical pharmacy practice experiences.
PharmD or Bachelor of Pharmacy degree from a college of pharmacy. Completion of a post-graduate residency or fellowship training is preferred. If specialty residency training is available in specialty area then specialty residency is preferred. Training in areas of clinical pharmacy related to specialty area of practice required. Current pharmacy licensure in the state of California. Board certification preferred. Completion of a post-graduate year 2 residency (PGY2) or fellowship or Completion of a post-graduate year 1 (PGY1) plus 3 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring in the specialty area or 5 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring in specialty area.Job tags
