Job description

Liaison between patients, families, visitors, volunteers, and hospital personnel. Functions as unit receptionists. Answers phone promptly, greet visitors and hospital personnel cordially. Works cooperatively with hospital personnel to ensure smooth functioning of the unit. Shows compassion towards patients and families, putting their needs first. Utilizes paging system, vocera and responds promptly to patient calls. Logs staff usage of vocera. Records and maintains an accurate call log.

Initiates and Coordinates Admissions, Transfers, and Discharges. Records information in the daily logbook as well as the patient record book without delay. Accurately including bed number, disposition and date and times. Ensures accuracy of admission book for statistical purposes. Removes all discharge patients from the system. Maintains awareness of bed status and communicates changes to admitting and the charge nurse promptly. Communicates need for bed cleaning to environmental services immediately upon patient discharge/transfer.

Transcribes physician orders with follow through to nurses and other appropriate departments. Transcribes physician orders accurately and promptly in the computer and scan orders to pharmacy. Notifies the RN of STAT and ASAP orders without delay. Ensures that medication orders are sent or faxed to the pharmacy with the correct patient’s name, account number, and bed assignment. Ensures that all specimens have been labeled properly, are entered in the computer, and are promptly sent to the appropriate department. Utilizes the correct downtime procedure when so advised. 

Maintains patient records, assembly charts, kardexes, and the appropriate nursing form for all admissions or all transfers in. Ensures that all forms are identified accurately, utilizing patient identification stickers. This includes the room number on the Doctor’s Order Sheet and any other form it is asked for. Maintain the charts in the proper order and post all reports promptly and in chronological order. Keeps patient board up to date in the Nursery, assignment books and census log on DA.

Initiates proper procedures and follows through on all equipment requests, repairs and returns to the unit. Communicates with the appropriate department for the purpose of obtaining material resources utilizing the PMM system. Collaborates with the Unit Manager regarding material needs. Charges for supplies as warranted. Keeps unit neat and organized. Replenishes bedside carts on a shift-to-shift basis as well as the PPE boxes or as needed.

Assist with activities of daily living. Utilizes the two patient identifiers when carrying out all direct patient care activities. Assist RN with bathing, skin care and nourishment. Assist RN in  turning and positioning patients, as well as inpatient transfers and/or ambulation. Administers incontinence care with special attention to skin care and communicating any redness to the RN. Provides ice water, juice, and snacks. Assist with Peri-care, fills linen carts/passes out linen. Utilizes whiteboard in patient rooms and keeps information current. Age Specific Competencies: Neonates, Adolescent, Young Adult and Middle Adult.

Assist RN with gathering vital patient information. Measures vital signs and records them in a timely manner. Reports vital signs outside normal parameters to the RN immediately. Performs and records chemsticks, and pulse oximetry. Reports findings outside of normal parameters to the RN.  Weighs patients and record findings.  Measures, and records foley catheters. Collect specimens as assigned, labels appropriately and communicate to the RN. Documents ADL’s, vital signs and I & O’s on the medical record in a timely manner. Assist with first time up to the bathroom Peri- care and ice packs support. Supports hospital wide and unit based each CAPS measure age specific competencies neonate adolescent Supports hospital wide and unit based HCAHPS measures. Age Specific Competencies: Neonates, Adolescent, Young Adult and Middle Adult.

Maintains compliance with infection control practices. Maintains universal precaution on all patients. With every contact washing hands thoroughly between each patient. Consistently observes isolation techniques as posted outside patient door or bed. Assist with sitz baths, remove, and reapplies veneodyne boots as necessary, and maintains baby under phototherapy. Documents voids and stools. Performs and records Dexstrostick. Discontinues foley catheter, discontinues IV. Cleans breast pumps, IV poles, pumps and phototherapy equipment as needed. Age Specific Competencies: Neonates, Adolescent, Young Adult and Middle Adult.

Observes role responsibility at all times. Demonstrates flexibility in responding to patient and family needs as part of our unit’s family centered/baby friendly care philosophy. Completes all assignments as directed by the charge nurse. Facilitates normal activity during episodes of crisis on the unit. Works as a team member in need and needs of the unit. Is flexible with scheduling requirements. Adheres to established dress code and hand and nails policy. Stays updated with organizational exceptions in order to maintain roll efficiencies. Orient new personnel as assigned stays. Proficient in all unit equipment. Troubleshoots computers and reports to help desk if necessary, keeping track of reference number until the issue is resolved. Attends required amount of staff meetings. Accepts and carries out other duties as required.
