Due to continued organizational growth, Liberty Healthcare is now hiring for several new full-time Assessor positions.

These are field-based roles which will allow you to partially work remotely from home and also require you to travel to client locations throughout the region.

As an Assessor with Liberty Healthcare, your responsibilities and everyday work will include:

• Learning how to administer the Supports Intensity Scale (SIS) assessment and demonstrating proficiency

• Leading in person and virtual meetings with people who have intellectual and developmental disabilities and/or autism, as well as their family members and caregivers

• Gathering and interpreting pertinent information from individuals and their families and caregivers

• Using standardized assessment tools to help determine needs for services and supports

• Completing structured reports within established deadlines

You are a good fit for an Assessor position with Liberty if you possess the following:

• Bachelor's degree in social work, psychology, special education, or related heath and human service field

• At least four years of experience supporting people who are on the autism spectrum or living with intellectual and developmental disabilities

• Solid verbal communication skills, including the ability to engage people in productive conversations

• Solid written communication, computer, active listening, and analytical skills

• Willingness to travel throughout the region

You must also be very organized, self-motivated, independent and able to be productive in an autonomous role.

You have relevant work experience and transferrable skills if you’ve worked as a Direct Support Professional, Social Worker, Healthcare or Human Services Intake / Admissions Specialist, Case Manager, Social Services Professional, Service Coordinator, Case Worker or Client Advocate.
