Job description
Are you a physician looking to expand your practice and gain experience in post-acute care? If so, TeamHealth has an excellent opportunity for you as a primary care physician in our skilled nursing facility in Lindenhurst, Illinois. We are seeking a physician who is board certified in internal medicine (IM) or family medicine (FM) with a passion to serve older adults.
Responsibilities: - Work alongside pharmacists on medication management
- Medicare required visits
- Acute and follow up visits as necessary
- Implement an individualized plan of care for patients' goals
