We are currently seeking a qualified and compassionate Medical Social Worker to join our team.

Position Overview:
As a Part-Time Medical Social Worker, you will play a key role in providing psychosocial support to our home health care patients and their families. You will assess the social and emotional factors affecting the patients' health and well-being, collaborate with the interdisciplinary team, and help coordinate community resources to support the patients' care plans.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments for patients and families to identify social and emotional factors impacting their health.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Provide counseling and emotional support to patients and families facing challenges related to illness, disability, or end-of-life care.
• Collaborate with healthcare professionals to address social determinants of health and enhance patient outcomes.
• Assist in coordinating community resources, support services, and referrals to meet the patients' social and emotional needs.
• Maintain accurate and timely documentation of patient assessments, interventions, and progress.
• Participate in interdisciplinary team meetings to discuss patient cases, treatment plans, and ongoing care needs.
• Advocate for patients' rights and facilitate communication between patients, families, and healthcare providers.

Qualifications:

• Master's degree in social work (MSW) from an accredited school of social work.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of 2 years of experience in medical social work, preferably in a home health care setting.
• Strong interpersonal and communication skills.
• Knowledge of community resources and support services.
• Ability to work independently and collaboratively as part of a multidisciplinary team.

• Part-time position with flexible hours.
