Overview
Under the direction of the Assistant Vice President of Nursing, coordinates all clinical inpatient pressure injury care/activities in collaboration with the Medical Director. Functions as a resource and educator for the clinical caregivers. Plans, directs and supervisors the workload of the certified Wound Care RN.
Responsibilities
- Coordinates and develops relationships with other sources of referrals and develops good networking with collaborating physician, nursing staff and medical staff.
- Collects data on patient outcomes and related data, and follows up to maintain relevant program statistics.
- Maintains program stats and Performance Improvement and reports accordingly to the Wound, Nursing PI and Quality of Patient Care committees.
- Answers and provides community with appropriate clinical information via phone, person and other.
- Develops and maintains policies and procedures related to wound care.
- Provides education to the patient/family/nursing staff regarding the wound care and planned treatment/intervention.
- Provides instruction in the areas of health promotion, activity and medication as appropriate.
- Coordinates the workflow and inpatient assignments in the department specific to pressure injuries.
- Conducts patient pressure injury assessments (staging) for stage 2, 3, 4, unstageable and DTI. Reassesses every 2-3 days.
- Orders appropriate treatments following evidenced based care/protocols.
- Demonstrates autonomy and ability to work independently and communicate patient issues to physicians.
- Coordinates the department's education/in-service schedule.
- Evaluates the patient's progress, goal attainment and need for further services following discharge.
- Makes appropriate referrals to the outpatient Wound Care Program and any other outpatient services.
- Tracks admitted Wound Center patients to the medical center to ensure high quality care.
- Completes a hand-off to the Wound Center at discharge.
- Evaluates the Wound Care RN on an annual basis and provides ongoing feedback during the year.
- Develops and monitors employee development program for each employee.
- Reviews budget on monthly basis with department head and on annual basis provides input for the budget for the following year.
- Ensures wound photos are completed on admission/observation patients, initial finding and weekly assesses and evaluates photos.
- Assumes performance responsibility for program and employees.
Qualifications
- Masters in Nursing (MSN) required
- 5 years of previous healthcare experience – adult or primary
- Licensed APN in NJ
- Licensed RN in NJ
- Wound Care certification must be obtained within one year of employment (CWS, WOCN)
- Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.
