General Summary:

The Physician Assistant or Advanced Practice Clinician provides care management services to medically complex gastroenterology patients. They serve as an advocate for patients and their families and assist them to navigate the health care system. The Care Manager facilitates the continuity of a patient's care by functioning as a liaison between EPGI, primary care providers (PCPs), specialty care providers and inpatient care teams, as well as payers, home care providers, subacute and chronic care facilities and community resources. They collaborate with ambulatory, emergency department and inpatient care teams with the goals of optimizing communication among care providers, promoting seamless transitions between care settings and facilitating the patients' medical care.

Essential Job Responsibilities:

• Report directly to the attending provider for patient review to obtain any required authorized signature on a patient's chart.
• Perform patient history and physical examination, regardless of complaint, recording pertinent information.
• Give concise written and/or electronic instructions to RN/LPN or appropriate staff.
• Make appropriate instructions for patient disposition as they relate to continuing care.
• For hospitalized inpatients, complete comprehensive report of patient consultation within 24 hours. Follow patient progress in hospital when director or when medically appropriate.
• For office outpatients, complete electronic medical record office notes within 24 hours of encounter and place in chart. Send correspondence to referring physicians when appropriate and in a timely fashion.
• Maintain complete professionalism verbally and physically at all times.
• Prescribe medications strictly in accordance with all Federal, State, and Local regulations.
• Maintain C.M.E. credits as per the appropriate professional society, or other appropriate regulatory agency guidelines.
• Attend and participate in resident conferences and teaching during regular work hours when directed.

Education:

• Current license as a Nurse Practitioner or Physician Assistant
• BCLS or BLS certification

Experience: At least 1 year of APC experience preferred

Mental/Physical Requirements: Light to medium duty. Requires frequent lifting and carrying items weighing up to 30 pounds unassisted, including assisting patients when required. Adequate physical ability including sufficient manual dexterity to perform the requisite health care service, including injections. Requires frequent bending, reaching, repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift. Requires exposure to communicable diseases, bodily fluids, medicinal preparations and other conditions common to a clinic environment.
