Overview

LifeLong Medical Care has an amazing opportunity for a highly motivated Registered Nurse Case Manager to join our multidisciplinary team providing exceptional primary care services at our Administrative Offices, Berkeley, CA. The RN Case Manager, in conjunction with the Cares Project Transition Case Manager, will provide chronic care management to identified high-risk patients of LMC's Lifelong CARES program. This position will improve quality of life by supporting pain and symptom management, assessing/addressing key risk factors and serving as a bridge and navigator between patients and the healthcare system.

The ideal candidate must be dedicated to providing high-quality primary care to patients with complex medical conditions and passionate about working collaboratively in a team model of care. As a key member of our team, you will work under the general supervision of the Transitions Manager to provide outstanding care and integrate whole-person care into our new team model. You will play a critical role in telephonic patient triage and chronic care management.

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Benefits:

Compensation: $82,000-$93,000/year, depending on years licensed, and excellent benefits, including medical, dental, vision (including dependent and domestic partner coverage), generous leave benefits including nine paid holidays, Flexible Spending Accounts, 403(b) retirement savings plan, and commuter benefits.

Responsibilities

Conduct patient assessment for high-risk patients to determine chronic care management needs
• Coordinate care with primary care provider, specialists, DME vendors, pharmacies and other service providers to facilitate smooth and coordinated
• Make referrals as necessary and follow through to ensure linkages are established and continue to meet the needs of the patient and family/caregivers.
• Provide education and counseling to patient and family.
• Coach patient family and care givers on goals of care discussions, an advanced care planning
• Problem-solve with patients, families/caregivers, and providers to address barriers to care, align care plans with patients' values, and support achievement of goals of care
• Coordinate need for psychosocial support with project Case Manager.
• Coordinate with hospital staff on discharge planning if hospitalize
• Medication reconciliation
• Provide post-hospitalization follow-up to be sure services are in place and patient receives continuity of medical care.

Qualifications

• Able to work independently and cooperatively in a multi-disciplinary team setting.
• Strong organizational, administrative, critical thinking and problem-solving skills, and ability to be flexible and adaptive to change while maintaining a positive attitude.
• Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally responsive.
• Demonstrated communication skills, both oral and written.
• Demonstrated ability and sensitivity working with people who have limited income and resources as well as complex health and/or social needs.
• Bachelor's Degree in Nursing.
• Current and valid California Registered Nurse license and BLS.
• Proficient in Microsoft Office Suite
• Electronic health records, Epic preferred.
• Reliable means of transportation for home visits and off-site work with patients
• Previous work providing services to persons with complex health and/or social needs, including any specific populations to be served
• Job Preferences
• 5 years' nursing experience
• Experiencing working in Hospice or palliative care
• Advanced working knowledge of outpatient care, triage and clinic flow.
• Specific jobs may require work/lived experience in a particular specialty area e.g. perinatal, homelessness, recovery, criminal justice, HIV/AIDS, palliative care and end-of-life care, or behavioral health

While performing the duties of this job, the employee regularly works in inside environmental conditions. The employee may be required to attend evening meetings, travel and work evenings or weekends. The noise level is usually quiet to moderate. Individual is subject to frequent interruptions.
