Overview

LifeLong Medical Care has an amazing opportunity for a highly motivated Registered Nurse Case Manager to join our multidisciplinary team providing exceptional primary care services at our Administrative Offices, Berkeley, CA. The RN Case Manager, in conjunction with the Cares Project Transition Case Manager, will provide chronic care management to identified high-risk patients of LMC's Lifelong CARES program. This position will improve quality of life by supporting pain and symptom management, assessing/addressing key risk factors and serving as a bridge and navigator between patients and the healthcare system.

The ideal candidate must be dedicated to providing high-quality primary care to patients with complex medical conditions and passionate about working collaboratively in a team model of care. As a key member of our team, you will work under the general supervision of the Transitions Manager to provide outstanding care and integrate whole-person care into our new team model. You will play a critical role in telephonic patient triage and chronic care management.

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Benefits:

Compensation: $82,000-$93,000/year, depending on years licensed, and excellent benefits, including medical, dental, vision (including dependent and domestic partner coverage), generous leave benefits including nine paid holidays, Flexible Spending Accounts, 403(b) retirement savings plan, and commuter benefits.

While performing the duties of this job, the employee regularly works in inside environmental conditions. The employee may be required to attend evening meetings, travel and work evenings or weekends. The noise level is usually quiet to moderate. Individual is subject to frequent interruptions.
