Job description

Yonkers Gardens Center for Nursing and Rehabilitation is now hiring an experienced MDS Assessor who is committed to providing the best life and care for our residents. Under the: direction of the Director of Nursing Services, the MDS / Care Plan Coordinator is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.

We are located at 115 S Broadway, Yonkers, NY 10701.

MDS ASSESSOR QUALIFICATIONS:

• Organizational Skills/Ability to Multitask
• Knowledge of PDPM/Clinical Concepts
• Ability to work with facility nursing management to identify and fix care documentation notes
