Job description
The Care Coordination Manager, is responsible for the effective case management of the census of the clinic they operate at. This includes overseeing day-to-day clinical case management for client-driven beneficial outcomes. The Care Coordination Manager will guide Case Manager caseloads and advise as necessary.
This position is responsible for the following:
- Serves as a subject matter expert for case management in support of the clients served; may provide one-on-one support for clients as needed; completes documentation in the EMR of client interactions as appropriate
- Creation of the Case Manager and Navigator schedules for proper coverage and support
- Creates and provide oversight for Case Management treatment plans to ensure timely and accurate documentation in the EHR; review all direct reports’ documentation for appropriateness and completeness of treatment-related information including case management notes, progress notes, correspondence, releases of information, etc.
- Communication with direct reports for support, delegation, progression, and training
- Provide guidance to direct reports; give guidance on decisions in compliance with policies and procedures
- Liaise with healthcare professionals, therapists, probation officers, and other professionals to ensure seamless communication and coordination of care across different services
- Communication with Clinical, Housing, Outreach, and administrative teams
- Participate in regular interdisciplinary staff meetings
- Develop and maintain collaborative relationships and communication with community referring agencies and community partnerships
- Stay updated with industry trends, policies, and advancements in care coordination practices
- Facilitate case management groups, as needed
- Other duties as assigned
The primary responsibility of the Care Coordination Manager includes managing Case Manager(s) and Navigator staff, including hiring, firing, training, appraising performance, and disciplinary action.

Position Qualifications
- Minimum of high school diploma or GED
- Minimum of 3 years of experience in working in the field of substance abuse treatment or behavioral health-related field with preferred case management experience
- AzAHP, Medicaid, Medicare experience strongly preferred - please highlight in applicationÂ
- Minimum 2 years prior supervisor/management experience
- Must be at least 21 years of age
- Clean driving record
- Complete all required documentation in accordance with company policies and state criteria.
