The Assessment Coordinator (RN) makes assessments for plans and advocates for the residents of the community facility so they can reach their highest practical level of physical, mental and psychosocial well-being. The Assessment Coordinator (RN) will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community.

Responsibilities
- Follows safety policies in performing nursing care, assuring resident safety and follows infection control guidelines.
- Attends and participates in facility meetings and in-service training programs as assigned
- Initiates emergency measures according to the community facility policy and within standards of nursing practice
- Maintains all documentation as required by Federal and State regulations, and the community facility.
- Promotes positive public relations with residents, family members and guests.
- Completes required forms and documents in accordance with company policy and state and/or federal regulations.
- Responsible for pre-admission assessment of referrals, coordinates admissions and discharge planning for all the residents
- The Nurse Admissions Coordinator coordinates and directs the care plan meetings.
- The Nurse Admissions Coordinator provides on-going education of resident's rights, abuse and neglect prevention to staff.
- This position must be willing to work the floor as needed as well.
Education, Experience and Requirements:
- Associate or Bachelor's Degree in Nursing is strongly preferred.
- Minimum one years' experience in Assisted Living, Alzheimer's, or Memory Loss care preferred.
- Experience with Microsoft Word and Excel preferred.
- Licensed in state of NJ and be responsible for maintaining professional licensure and CEU requirements.
