Job description
Position Summary:
The Pharmacist coordinates and provides pharmaceutical care for patients under the guidance of the Director of Pharmacy. They are responsible for managing drug therapy protocols as part of an interdisciplinary team reviewing medication orders providing drug information and education documenting interventions and ensuring the accuracy and completeness of medication orders.
Reports to: Director of Pharmacy
Responsibilities of the Position:
- Manage drug therapy protocols in collaboration with the interdisciplinary team.
- Review interpret and evaluate medication orders to identify prevent and resolve potential or actual drugrelated problems.
- Provide drug information and education to medical staff healthcare professionals patients and pharmacy students.
- Document interventions and their outcomes for use in drug therapy quality improvement efforts.
- Evaluate medication orders for accuracy and completeness before order entry and after filling by technicians.
- Maintain adequate knowledge of pharmacy automated systems and computer software.
- Perform other tasks and duties as assigned by the Director of Pharmacy.
Requirements
Minimum Education:
- Doctor of Pharmacy degree is required.
Certifications/Licenses:
- Active Pharmacist License is required.
- Basic Life Support (BLS) certification through the American Health Association is required within 30 days of hire.
Required Skills:
- Critical thinking skills decisive judgment and the ability to work with minimal supervision.
Five years of nursing experience, including a minimum of one year in nursing management. Experience in budget management, cost reduction, and forecasting operational needs. Communication and Stakeholder Engagement: Excellent communication skills, with the ability to engage and collaborate with staff, physicians, and other stakeholders. Promote staff ownership in problem-solving and operational decision-making. Pharmaceutical Expertise: In-depth knowledge of pharmaceutical services and the delivery of drug-related information in both inpatient and outpatient settings. Stay up-to-date on industry developments, incorporating innovative systems while ensuring compliance with regulatory standards.
