Overview
Dignity Health Home Health services provide comprehensive care for patients who have recently been discharged from the hospital or homebound individuals needing professional nursing and/or rehabilitative care. Our services and specialties include: Chronic Disease Programs and Specialty Care, Largest Rehabilitation Team on the Central Coast, Largest Wound Care Team on the Central Coast, Maternal and Infant Services, Medical Social Workers, Nutritional Counseling, Pediatric & Neonatal Care & Skilled Nursing.
ResponsibilitiesIn keeping with the Dignity Health core values the Speech Language Pathologist is responsible for providing speech language pathology therapy assessments and treatments to patients in the home setting. Provides training to patients family members and caregivers. Participates in orientation of new team members. Interacts with multidisciplinary teams for coordinated patient care. Actively participates departmental meetings and performance improvement activities as requested.
QualificationsMinimum Requirements:
Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire. This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.
