CVHCare, a leader in Home Health Clinical services , headquartered in beautiful San Ramon CA,is currently seekinga Registered Nurse to join our Home Health Care Agency. Schedule: Part Time - 18 RVE p/week (comes with benefits) and Per Diem (12 visits p/week)
* Non-traditional schedules are available, i.e., Thursday-Monday
Locations: Area #13 covers the following cities: Atherton, Belmont, Foster City, Half Moon Bay, Menlo Park, Montara, Portola Valley, Redwood City, San Carlos, San Mateo, Woodside, El Granada, La Honda, Loma Mar, Moss Brach and Pescad ero.

RN monitors a patient's overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager, and works independently to deliver quality healthcare to their patients. As well as customized care and patient education about disease management, the Home Health RNs provide a variety of specialized services, such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management.

We provide an entire internal operational support team for our clinicians to handle the tasks of pre-populating the medication reconciliation, tracking down DME orders, getting MD signatures, coding, lab follow-up, etc., truly allowing you to function independently, with the flexibility to make your own schedule and plan patient visits around other commitments if needed. The home care setting allows you quality time with your patient in the comfort of their own home.

Minimum Required Qualifications:

• Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting
• Must Possess an active CA Registered Nursing License
• AA, BSN, MSN
• Reliable Transportation
• Current CPR or BLS Card
