Home Care Team Manager
Are you looking for a leadership role that can truly make a difference in someone's life, then come join the Mystic Valley Team! The Home Care Team Manager position is an opportunity to guide case managers as they build relationships with older adults and adults with disabilities in the community.
Schedule: This is a full-time, exempt (35 hour/week) position. We offer 4-day work week and hybrid work schedule (once training is complete.) No nights or weekends.
Compensation: Starting annual salary is $69,000. Being bilingual is not a requirement for this role, but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish, Haitian Creole, Russian, Vietnamese, Cantonese or Mandarin. In this role you will be responsible for assessing, reassessing and monitoring consumers for need and eligibility for in-home services, recommending/approving service plans, providing information on available resources, and coordinating in-home/community services.

Job Activities
Job activities of the position are below. Additional duties may be assigned as required.
- Provide consultation and serve as a resource for assigned team and to others in the Home Care department in the absence of their designated supervisor.
- Monitor case records for accuracy, completeness, and continuity of the consumer's status.
- Ensure that quality assurance and customer service standards are met.
- Encourage staff development through supervision and training.
- Provide supervision for employees who are seeking or have obtained their social work license.
- Evaluate individual staff performance and hold them accountable for meeting standards.
- Identify and address any performance issues.
- Provide crisis care management support as appropriate.
- Promote and support team model and interdisciplinary care management.
- Participate in the interviewing and selection process for hiring new staff.
- Participate in the orientation and training process for new staff.
- As a member of the supervisory team, assist the Director of Home Care in ongoing department planning and development.
- Participate in problem solving of management issues and discussion of care management and policy-related issues.
- Serve as a liaison with the human service network in the communities served.
- Participate in task oriented committees.
- Develop and maintain programmatic reports as required.
- Maintain Home Care policy and procedure manual.
- Coordinate and assist in the development of new initiatives in the Home Care department.
Qualifications :
- Master's degree in social work or related field plus 1 year of supervisor experience
- 2 years of field experience preferably with older adults
- Strong leadership and managerial skills.
- Problem-solving skills.
- Strong interviewing and assessment skills.
- Strong interpersonal and organizational skills.
- Excellent communication skills, both written and verbal.
- Ability to effectively facilitate meetings.
- Proficient with MS Office Suite and entering data into a database. Working knowledge of Excel. Ability to use the Internet to conduct information searches.

Preferred Qualifications :
- Knowledge of and experience working with older adults and adults with disabilities.
- Experience working in a community service setting.
