Job description
Wood County Hospital is accepting applications for an RN Care Coordinator.
Job Duties:
Responsible for conducting patient assessments and ensuring completion of referrals to applicable parties, high-risk screenings, and crisis interventions.
Reviews patient records for medical necessity, level of care, length of stay, and authorization denials for inpatients and observation patients.
Collaborates with the healthcare team to coordinate care according to evidence-based, approved guidelines. Develops and facilitates care plans throughout patient hospitalization and episode of care.
Provide services which support optimal transitions of care. Assists in the clinical appeal process.
Shift Details:
Hours: Full Time - 40 hours per week
Shift: 7:00am - 3:30pm
Weekend rotation and On-Call requirements.
Minimum Qualifications:
Minimum Education:
Minimum Certification:
Licensed RN in the state of Ohio
BLS certified through the American Heart Association
Minimum Experience:
2 years previous experience in UR and Case Management.
2 years of acute care nursing experience required.
Experience utilizing InterQual and MCG preferred.
