Job description

We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to join our interdisciplinary team (IDT). As a support RN, you will play a vital role in delivering high-quality hospice nursing care to our patients. This position, located in Lawrence, MA, with a potential for remote work, involves a commitment of 24 hours every weekend from 7:00 am to 7:00 pm.

Minimum Qualifications:

Massachusetts RN Licensure.
New Hampshire RN Licensure.
Basic Life Support (BLS) certification.
One (1) year of hospice and home health experience.
Duties and Responsibilities:

As a Hospice RN, you will be responsible for:

Providing hospice nursing care with a focus on palliative care and symptom management.
Coordinating care efficiently to meet patient needs.
Collaborating effectively with other members of the Interdisciplinary Team.
Using the nursing process and current standards of practice to deliver hospice care.
Communicating with physicians and our Tufts Medicine Care at Home staff regarding changes in a patient's condition.
Understanding family dynamics and adapting to various environments.
Acting as a patient and family advocate, providing information about available resources.
Addressing the need for additional services accurately.
Performing an adequate number of triage calls to ensure superior hospice care under Tufts Medicine Care at Home standards.
Routinely updating Care Plans, Medication Profiles, and Treatment Plans based on changes in patient conditions.
Completing all nursing documentation per procedures and nursing standards of practice.
Involving the Clinical Manager and Medical Director appropriately in challenging situations.
Collaborating with the Clinical Manager to identify opportunities for performance improvement and actively addressing them.
