Position Summary: The Registered Dietitian is responsible for the standard Registered dietitian responsibilities in addition to disease management programs– success is based on outcomes of the patient’s health results. The Registered Dietitian is responsible for counseling members, supporting providers and conducting a robust calendar of activities to promote the adaption of healthy behaviors, appropriate use of the healthcare system and assisting patients to adhere to their healthcare provider’s treatment plan.
Essential Duties and Responsibilities include the following: - Deliver a wide range of services, including providing quality and documentation, providing scheduling maintenance, and promoting the delivery of health education services.
- Provides direct group or one-on-one health education services to members and the public.
- Reviews, develops, and recommends high quality, culturally appropriate written, audio, and visual health education materials.
- Consults with physicians and other staff regarding related nutritional services.
- Assist with executing health information projects such as program flyers, websites, newsletters, informational displays and health events.
- Participates in specialized specific area of health education (e.g. prenatal, chronic disease, health promotion, diabetes) as required.
- Synthesizes data and prepares monthly activity reporting.
- Adheres to established work schedules and practices effective time management.
- Assist with reports, proposals, and documentation as assigned.
- One -on- One clinical nutrition education: Provides high quality, evidence-based, efficient, patient-focused education, including nutrition screening, assessment, care planning and education for appropriate intervention.
- Develops, implements and monitors ongoing plan of care based on the screening and assessment of the patient specific care plans/education and based on the patient's age, capability and psychosocial status, diagnosis, cultural background and religious affiliation.
- Prior experience in health coaching disease management and/or patient education with emphasis on behavior change techniques, adult learning theory, group process theory and application, with a strong understanding of disease management principles.
- If Medical Nutrition Therapy is performed, under California Law, this will be performed by only a Registered Dietitian - If the position requires a Registered Dietitian then this section applies: - Individual Medical Nutrition Therapy (MNT) - Provides high quality, evidence-based, efficient, customer-focused MNT, including nutrition screening, assessment, care planning, education for appropriate nutrition intervention
- Strong interpersonal, written communication skills, provide information and resources and teaches skills to members to better manage chronic diseases and impact overall wellness.
- Knowledge of PC applications required, demonstrates working knowledge of Windows
- Experienced in curriculum and materials edit and development, required
- Other duties as directed by management.
The pay range for this position at commencement of employment is expected to be up to $45 per hour depending on experience and license; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements Education and / or Experience: - Must be a Registered Dietitian (RD), as confirmed by the Commission on Dietetic Registration.
- Bi-lingual, Bi-literate (English and Spanish) preferred.
- Five years prior experience in healthcare setting with knowledge and skill in facilitating health education practices, group and one- on - one health / Nutrition education.
- Keeps all certifications and licensures current
- Minimum of 5 years’ experience and expertise providing diabetes self-management or other chronic disease management and education.
- Experience developing and implementing healthy eating strategies specifically for older adults and underserved populations.
- Computer proficiency in use of Word, PowerPoint, Excel and Internet.
- Must have California driver’s license, use own vehicle for business related travel and be responsible for all liability on vehicle.
- Must be physically mobile and able to communicate verbally and electronically.
