Talent Pool: Professional Management

Position is responsible for oversees the THN clinical PAC program/process. The role is accountable to develop and maintain clinical PAC process that align with THN strategic goals. This position is responsible for clinical PAC Network performance management, development of clinical PAC infrastructure and support tools. This role is responsible for report out on financial performance of clinical PAC Network and recommend/implement changes.

 

Develop close working relationships with teams at key strategic post-acute partner sites and community practice-sites. Ensure adherence to efficiency and quality targets for THN ACO patients in partnership with PAC team. Collaborate with acute care facility social work and CM staff to identify opportunities to support the PAC work.

Identify to drive appropriate utilization targets in conjunction with PAC team. Assess, plan, implement, coordinate, monitor and evaluate the options and services needed to meet the PAC patient’s health needs across the care continuum and provides quality cost-effective outcomes.

Develop relationship with Home Health, SNFs, Hospice and other community partners through scheduled meetings, working on pilot projects, telehealth training and sharing quality outcomes

Participation in any applicable events and/ or projects related to the PAC THN ACO members. Attends various THN committee meetings related to PAC performance, and prepares reports and presentations to leadership
Work with the Director and Executive Director of PAC to develop cost savings and quality improvement projects. Assist in establishing Post-Acute Care preferred provider network to impact THN long term.


 

EDUCATION:

Required:
Bachelors in health related field.

 

EXPERIENCE:

Required:
Understanding PAC and experience in managing multi SNF& HH engagement
Ability facilitate/develop clinical standards and workflow
Strong written and verbal communication skills
Strong skills in using various technology for additional communication
High degree of emotional intelligence, assertiveness, and ability to develop and strengthen collaborative team based relationships to meet defined outcomes
Experience with knowledge of project planning and ability to coordinate various projects.
Excellent organizational and prioritization skills; ability to lead multiple processes at the same time and obtain desired outcomes on schedule.
Professional attitude.
Strong communication and presentation skills required.
Interpersonal skills necessary in order to interact effectively and build strong relationships with community partners, physicians, staff and all levels of management.
Analytical skills necessary to identify and oversee community partner outcomes in order to develop work plans.
Working knowledge of Microsoft Office – Excel, Word, and Power Point

 

LICENSURE/CERTIFICATION/REGISTRY/LISTING:

Required:
Valid Driver's License
If driving Cone Health vehicle, must have 5 years of driving experience and MVR must be approved by Risk Management.
