This is an excellent opportunity for a highly motivated Diabetes Clinical Pharmacist to work with both The Everett Clinic and The Polyclinic to manage adult diabetes patients within the Primary Care and Endocrinology teams to support high quality patient care. This is a full-time position with variable hours Monday – Friday.  No weekends or call. 

Primary Responsibilities:

• Embedded within the care team to provide high quality care with a focus on the care of adult diabetic patients (Type 1 and Type 2)
• Provide and direct patient care based on evidence-based guidelines, protocols. 
• Performing chart reviews and provide disease state management via. collaborative drug therapy agreement (CDTA).
• Potential to perform patient medication reconciliation, medication therapy management (MTM), medication optimization and 5-star quality adherence work.
• Work with providers and clinical teams to implement cost-effective medication interventions.
• Provide clinical medication education and feedback for providers.

Required Qualifications:

• Doctorate in Pharmacy 
• 3 + years of experience in Type One Diabetes management in a non-retail setting
• 1-2 years of direct patient care experience
• Working under a collaborative drug therapy agreement
• Licensed pharmacist within the state of Washington or the ability to obtain a WA license

Preferred Qualifications:

• Board certification by the Board of Pharmacy Specialties (BPS)
• Residency trained
• BCACP – Board Certified Ambulatory Care Pharmacy
• CDCES (Certified Diabetes Care and Education Specialist, formerly known as Certified Diabetes Educator – CDE)
• BC-ADM (Board Certified in Advanced Diabetes Management) preferred
• EPIC experience highly preferred

Washington Residents Only : The salary range for Washington residents is $143,000 to $167,000.
