JOB SUMMARY

RN Case Manager

The Registered Nurse (RN) Case Manager coordinates and advocates for the patient’s care. The RN Case Manager will facilitate the appropriate use of resources, effective planning, and coordination of services for patients to help ensure they are receiving the appropriate level of care.

The RN Case Manager will assist in removing pertinent logistical burdens to ensure patients are afforded time and space to focus on their health.

PRIMARY FUNCTIONS
  1. Provide healthcare case management to individuals through coordination with the patient, health care providers, and families
  2. Compassionate, motivational, and able to relate to different clients with various needs
  3. Strong verbal and written communication skills to explain to clients, family members, friends, and professionals the case and care plan
  4. Critical thinking and problem solving to determine the best plan of care for each client.
  5. Organization to manage several different cases at once
  6. Computer literacy to maintain, and manage good case records
  7. Conduct intake assessment with all new referrals
  8. Develop individualized care plans that account for physical, financial, and other constraints and update as needed
  9. Provide ongoing assessment, planning, implementation, and evaluation of patient’s progress
  10. Evaluate patient’s treatment plan for appropriateness, medical necessity, and cost effectiveness
  11. Ability to utilize nursing knowledge to allow the case manager to discuss the current treatment plan with the provider, and possible alternate treatment as needed to improve outcomes
  12. Negotiating and coordinating the delivery of durable medical equipment and/or nursing services
  13. Other duties as assigned.
EDUCATION
  1. Licensed RN in Florida
  2. Bachelor’s degree in nursing, preferred
  3. Advanced degree in nursing strongly recommended
  4. Certification in community and/or hospital case management recommended
  5. 1+ year’s case management experience; 3+ years preferred. 2+ years in a customer service focused environment within a similar healthcare facility
EXPERIENCE
  1. Strong understanding of ethics surrounding the storage of patient information
  2. Exceptional leadership, supervisor, and motivational skills
  3. Committed to promoting patient’s well-being
  4. Knowledge of/experience working with local social services, public health organizations and resources.
ADDITIONAL QUALIFICATIONS
  1. Bilingual a plus (English & Spanish and or Creole).
  2. Up-to-date immunizations
  3. Valid Driver’s License
  4. Must be highly organized, detailed, and reliable.
  5. Excellent written and oral communication skills
REPORTING RELATIONSHIP
  1. Reports to the Quality Manager
