Job description
Integrative Health is seeking experienced Advanced Practice Providers (FNP/PA) to join the team in The Woodlands, Texas. Full-time or Part-time.
Criteria: Board Certified FNP or PA with active Texas license, TX DEA with background in EM, UC or Primary Care.
The Transitions of Care Clinic offers patients a follow-up visit after leaving the emergency room, or after a hospital stay, that acts as a bridge between the hospital and the patient's long term care plan.
The candidate must have the ability to create a personalized care plan for patients that can include the following:
Office Hours: Monday - Friday, 9a - 6p
Compensation: Guaranteed hourly rate of $80 with paid malpractice insurance. Benefit options available for full-time.
