The Pharmacy Fulfillment Coordinator/ Care Management Support Assistant 2
works as a liaison between case managers, doctors, nurses and patients as they transition on service with One Infusion Pharmacy. Referral coordinators are responsible for coordinating care with external and internal departments to ensure accuracy and completeness of all pertinent clinical, billing and demographic information in accordance with the policies and procedures of One Infusion Pharmacy, state regulations, and other regulatory entities such as Accreditation Counsel for Home Care (ACHC). The referral coordinator works under the direct supervision of a licensed pharmacists and assists with coordination and delivery of products and services to One Infusion Pharmacy's patients. Patient registration, onboarding and coordination of care will comprise at least 50% of the daily tasks for the referral coordinator
