Job description
Avisa Recovery is seeking a compassionate and dedicated Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive case management services to clients in our care.
Your primary responsibilities will include:
- Assessing clients' needs and developing individualized care plans
- Monitoring clients' progress and adjusting care plans as needed
- Collaborating with interdisciplinary team members to coordinate and integrate services
- Assisting clients in accessing and utilizing community resources
- Providing education and support to clients and their families
- Maintaining accurate and up-to-date documentation
We are looking for an individual who is organized, detail-oriented, and has excellent communication and interpersonal skills. Experience working in a similar role and familiarity with mental health and substance use disorders is preferred.
Requirements

- Bachelor's degree in social work or a related field (preferred)
- Experience in case management or a related field (preferred)
- Strong communication and interpersonal skills
- Ability to work effectively as part of a team
