Job description
Join us as a Primary Care Advanced Practice Provider at Chestnut’s federally qualified health center and become an integral part of our patient-centered, integrated healthcare environment. This position requires an RN with a master’s degree in nursing from an accredited university and Advanced Practice Nurse (APN) or Advanced Practice Registered Nurse- Full Practice Authority (APRN-FPA) license or PA degree from an accredited university and Physician Assistant in Medicine license in the state of Illinois. As a Primary Care Advanced Practice Provider, you'll provide high-quality, culturally sensitive primary care services to pediatric through geriatric patients, working in an integrated primary care and behavioral health environment.
As a Primary Care Advanced Practice Provider, your responsibilities are multifaceted, encompassing comprehensive patient-centered care. You'll conduct thorough assessments, develop treatment plans, and provide direct medical services adhering to state regulations and agency policies. Collaboration and participation in program development activities, utilizing CFHC-approved Clinical Protocols, and consistent adherence to quality, productivity, and documentation standards are key aspects of this role. You'll play an active role in ensuring compliance with all applicable standards and promoting a culture of customer service excellence.
