Job description
Home Health Registered Nurse
Home Health RN Responsibilities: - Assess patient to determine skilled home health care needs
- Develop a Plan of Care to address patient's needs
- Re-assessments and changes to Plan of Care as necessary
- Supervision of caregiver staff assigned to the patient
- Patient and family education for safety, best care outcomes, and to prevent hospital re-admissions
- Develop discharge planning needs when goals have been met
Requirements: - Valid & current RN license
- Skilled Home Health and OASIS experience preferred
- Acute Care experience preferred
- Ability to learn EMR software
