$29.47/hr minimum - $44.21/hr maximum (range increases based on education, experience and certifications)
Position Summary• Responsible for the day to day activities of case management. Performsconcurrent patient assessment related to appropriateness of the level of care,diagnosis, procedures performed, discharge needs, and intervenes to expedite thepatient’s plan of care.
Education• Associate’s degree in NursingLicenses and Certification• Current Illinois RN LicenseExperience and Skills• Technical Experience: 3 years
