Job description
Atlantic Health System is seeking a part-time Socical Worker for the Int Med Fac Assoc. Suite 210 located in Morristown, NJ.
Essential Functions:
- Provide Social Work coverage to facilitate discharges, meet with patients and families to assess and provide discharge planning support, resources, referrals, transfers to Extended Care and inpatient psychiatric facilities.
- Work closely with the Healthcare Team to implement the plan of care, obtain insurance authorizations as required, communicate with community resources and area facilities.
Education:
- MSW with current NJ State Social Work License, LSW, LCSW
Minimum Experience Required:
- Communication skills, problem solving, organization, ability to work with a team, compassionate service delivery, ability to work with all age groups
