Overview
St. Luke's Health System's Home Care team is looking for a part time (16 hours a week) RN to join our Treasure Valley team!
Hours: Days of week vary - Mon - Fri from 8:30am-5:00pm with rotating evening call and weekends.

The Home Care team will help patients and loved ones be as comfortable and independent as possible by creating a custom plan that offers the latest technology and is tailored the patient and family’s needs. Become a part of a dynamic, interdisciplinary team comprised of Case Manager RNs, Rehab Therapists, Respiratory Therapists, Dietitians and aides in the collaborative effort to provide exceptional patient-centered care!

Responsibilities
You will provide comprehensive care to patients with a new diagnosis, discharged from ER, new medications, oxygen in home requiring respiratory treatment, and wound care. During home visits, you will ensure patients are progressing with their care plan, provide education related to their disease process, and assess physical and psychological needs to improve quality of life. When it comes time to discharge, you will coordinate resources and educate patient and patient's family.
The Registered Nurse is responsible for evaluating patients and assisting physicians with patient care. Plans, implements, and documents nursing care based on patient’s needs.
Identifies patient needs and determines priorities for care through assessment, planning, intervention, and evaluation.
Develops, implements and coordinates individualized plan of care for patients assigned through initial and on-going assessments.
Initiates assessment based on the patient's problems or needs, the physician's medical diagnosis, and the functional health framework.
Performs therapeutic nursing interventions.
Manages care of patients.
Documents care based on standards of care and practice, patient acuity, and/or clinical priorities.
Advocates for evidence-based practice care standards and supports adoption of evidence-based practices and innovations in care delivery.
Provides patient centric care in alignment with professional nursing practice model.
Other duties and responsibilities as assigned.
Qualifications
Education: Associate's or Bachelor's of Nursing Degree
Experience: At least one year of RN experience required.
Licenses/Certifications: Current RN licensure from the State of Practice; AHA-BLS Certification
Valid driver's license and current auto insurance for in home visits. You will be required to use your own vehicle while in the field.
