The Physical Therapist provides rehabilitative care (physical therapy) for home health patients needing assistance and training in ambulation, transferring, gait training and home exercise programs. The Physical Therapist works as part of the home care team including RN, PTA, OT, SLP, MSW and Home Health/Hospice Aides. The physical therapist also provides palliative physical therapy services to VNA Hospice patients on an as-needed basis, in accordance with the hospice plan of care.
Provide physical therapy services to home health patients according to a plan of care. This may include, but is not limited to:
- Assess and evaluate therapeutic/rehabilitative/functional status and participate in developing the total plan of care.
- Assess for muscle strength, mobility, gait, range of motion (ROM) and transfer capabilities.
- Direct physical therapy treatment.
- Instruct patients, families and caregivers in the use and care of therapeutic appliances.
- Determine priority needs for physical therapy.
- Report patient’s reaction to treatment or changes in care to patient’s physician as needed and appropriate.
- Conduct appropriate evaluative procedures, assess patient’s home environment and identify patient’s equipment needs.
- Provide physical therapy services to hospice patients through such modalities as: pain relief through the use of heat and cold; use of equipment and safety modifications promoting ADL’s, and in turn helping the patient retain a degree of dignity, independence, and control in order to enhance their quality of life.
- Initiate physical therapy program and instruct other personnel, patients, families and/or caregivers in the specified therapy program, as well as the goals of therapy.
- Participate in patient case conferences.
- Confer with the attending physician, home care or hospice team members and patient, family and/or caregivers to promote coordinated and comprehensive care.
- Communicate the plan of care to the attending physician, home care or hospice team members and patient, family and/or caregivers.
- Participate in meetings, in-services and other related activities as needed.
- Prepare and submit clinical and progress notes in a timely manner and in accordance with Medicare Home Health and Hospice Guidelines, licensure laws, VNA of Hanover & Spring Grove policies and procedures and all related state and federal regulations.
- Instruct patient and/or family/caregiver and other team members in the patient’s treatment regime as indicated.
Qualifications:
- Must possess a bachelor’s or master’s degree in physical therapy approved by an accredited organization: The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, The Council on Medical Education of the American Medical Association and The American Physical Therapy Association.
- Minimum of two years experience as a physical therapist. Home Health experience preferred.
- Must possess current license as a Physical Therapist from the Commonwealth of Pennsylvania or made application for same.
- CPR certified.
