Position Summary
The Sisters of Notre Dame de Namur (SNDdeN) is a Roman Catholic religious congregation whose focus is to make known the Goodness of God, especially among women and children. The Lead Healthcare Coordinator will support Sisters and assist with the medical issues of those living outside of our SNDdeN health facilities in New England. It involves being a support and resource to Sisters, particularly with Sisters receiving a significant medical diagnosis and the possible life changes it may bring. It also entails being a support and a resource person to the community coordinators and advocates in our facilities. She will carry out her duties in the spirit of the SNDdeN Wellness document, a holistic healthcare guideline.
Responsibilities
The Lead Healthcare Coordinator (LHT) is directly responsible to the representative of EW Leadership. The Lead HealthCare Coordinator has three primary areas of responsibility: 1) providing care services to Sisters of Notre Dame de Namur who are not in our healthcare facilities; ensuring that appropriate health and psychosocial services are available to and utilized by individual members; 2) facilitates admissions to Notre Dame Long-Term Care (LTC) as the SNDdeN LTC Liaison ; 3) providing professional supervision of North East (NE) healthcare coordinators and assistants, reflecting the values of the Sisters of Notre Dame de Namur.
In this role, the Lead Healthcare Coordinator is responsible for developing and nurturing the professional competency of the NE Healthcare Coordinators and assistants, ensuring that each delivers the expected quality of services for their respective Sisters.
- Assure access to health care and pastoral support for Sisters
- Work closely with Support Coordinators to assist with Sisters' health needs
- Communicate effectively with support coordinators, community coordinators, and advocates
- Sisters are encouraged to maintain their own medical information. PCPs and hospitals have online sites such as My Chart and Patient Gateway where sisters can obtain all of their medical information
- Perform simple medical treatments when needed
General Duties
Work in collaboration with the other healthcare coordinators and assistants to assure the following:
- Sisters are accompanied to medical appointments when necessary
- Coordinate appointments when necessary
- Liaison with medical and ancillary services as needed
- Transport Sisters to/from hospitals and rehabs as needed
- Visit Sisters in hospital as needed
- Work with case managers to provide safe discharge plans
- Pick up medication when necessary
- Educates the sister re her diagnosis, her medications, and her plan of care.
- Keep the Support Coordinator updated on the Sister's general condition
- Attend Interdisciplinary Meeting (IDM) and Care Plan meetings of Sisters in hospital or rehab centers
Rehabilitation Services
- Communicate with appropriate personnel regarding Sisters coming to Ipswich or Long-Term Care for recuperation
- Obtain necessary information from hospital case managers, ensuring that medications are ordered and Visiting Nurse Association (VNA) services have been scheduled
- Visit Sisters in recuperation and keep them updated on their progress with VNA
- Fill out the recuperation forms completely and pass them to RCC along with the requested insurance formation and community contact information
- Accompany Sisters in recuperation rooms to follow-up appointments
- Communicate updated orders from each appointment to the nursing staff and pick-up medications as necessary
- Provide or arrange transportation for Sister when she can return to her Community
- Keep in contact with Sister as necessary
Facility Coordination
- As SNDdeN Liaison, coordinate the Admissions process of Sisters into Long-Term Care directly with the Director of Admissions, following existing procedures
- Work with out-of-state Sisters and their local healthcare and support coordinators for admission to Notre Dame Long-Term Care
- Contact the Province Healthcare Benefits Manager regarding insurance needs for any financial information that may be needed on the LTC Application
- Coordinates the admission process for Sisters moving to Julie House, Notre Dame du Lac, St. Julie Billiart Residential Care Center when directed by a support coordinator
- Facilitate admission to the recuperative center at Ipswich for a sister requiring short-term recoup time following a hospitalization prior to returning to her community, following existing procedures
The Lead Healthcare Coordinator is expected to:
- Coordinate the schedules of healthcare coordinators and assistants
- Be available for emergencies and to work every other weekend
- Plan regular meetings with the NE Healthcare Coordinator and Assistants
- Participate in scheduled meetings with Support Coordinators
- Participate in quarterly meetings with NE Healthcare Coordinators, Advocates, Support Coordinators, Community Coordinators and Team Liaisons
- Prepare and monitor a budget
- Delegate responsibilities as needed
Preferred Skills/Education/Experience
- 4-year nursing degree preferred OR three years' experience as LPN, RN, medical social worker or equivalent experience working in geriatric case management. A current Massachusetts License
- Knowledge of aging issues, dementia healthcare service, and long-term healthcare models
- Respects and Maintains confidentiality
- Interest in working with Catholic Sisters with health needs
- Understanding and application of HIPAA regulations
- Ability to communicate with Sisters of all capacities regarding medical issues and pastoral care
- Detail-oriented and able to communicate with health care professionals clearly
- Ability to drive, including at night, and assist passengers
- Computer competency, especially in Microsoft Office programs (Word, Excel, etc.)
- Good organizational and planning skills
- Reliable - a sense of humor - flexible
