The LDH OPH Pharmacy Services Pharmacist 3 position is responsible for providing pharmaceutical services in support of LDH-OPH


Programs through local Public Health units and clinics statewide. The incumbent to this position represents Pharmacy Services to LDH OPH staff and the citizens of Louisiana and is expected to regard customer service as a priority in all that they do. Current licensure in good standing with the Louisiana Board of Pharmacy to practice pharmacy and a minimum of 2 years practical experience is required.


40% Replenish stock medications utilized by the Public Health units following the Pharmaceutical Replacement SOP. Review and |interpret clinicians ' per patient prescriptions and orders; enter all pertinent patient demographics and drug data into the pharmacy prescription filling system; reconcile prescriptions and orders with computer profiles to detect problems with dosages, indications, duration of therapy, allergies, drug/drug interactions, drug/food interaction, adverse drug reaction, duplicate therapy. Verify/double check all finished prescriptions/orders for completeness, accuracy and for compliance with all federal and state regulations including OBRA and HIPPA laws prior to dispensing. Offer to counsel patients regarding appropriate utilization of medication including drug name, dosage, proper directions and use, potential drug/drug interactions, proper storage, expiration date, side effects, adverse reactions and need for patient to consult prescribing clinician to report/monitor problems; print and offer to discuss Patient Information Leaflet (PIL) with patient or caregiver. Consult with prescribing clinician (physician or nurse) or notify of potential problems and or discrepancies; document in computer actions as a result or outcome of consultation with clinician. Utilize the automated dispensing machine including calibrating, loading, and operating system to prepackage stock medications as applicable. Prepackage medications for replenishment of stock medications used at the Public Health Units. Package prescriptions/orders for shipment to Public Health Units and place in designated area at appropriate times.


40% Submit claims for prescription reimbursement to third party insurance payers including Medicaid and private insurance. Perform functional supervision of pharmacy technicians and clerical staff as it pertains to operating the pharmacy in accordance with Louisiana Board of Pharmacy and Federal regulations.


10% Perform daily and monthly backups of all computer files and data; perform Drug Use Evaluation (DUE) studies. Perform quality assurance activities including data collection, analysis and interpret the resulting changes to improve operations. Perform professional level pharmacy duties involving efficient procurement; proper storage, appropriate control and accountability and effective drug distribution and related supplies. Perform inventory control including checking stock levels, preparing lists of drugs and supplies to be ordered, receiving drugs from pharmacy warehouse and vendors and verifying all drugs are received as ordered. Notify proper personnel to order replacement supplies and medications.


5% Complete monthly total prescriptions filled report by Program on the last working day of the month and turn in to front office personnel. Update/print and maintain daily and monthly prescription logs.


5% Satisfactorily complete all assigned training courses by completion deadline. Participate in bioterrorism and emergency preparedness drills, meetings and events; provide pharmaceutical care to patients in conventional and non-conventional clinic settings as well as other healthcare initiatives as needed. In the event of a disaster or emergency in the State of Louisiana, assists in the Emergency Operations Center or other location as assigned. Accept other duties as assigned by supervisors.

Qualifications



MINIMUM QUALIFICATIONS:


Possession of a current Louisiana license to practice pharmacy plus two years of experience as a licensed pharmacist.


SUBSTITUTIONS:


A master's degree in pharmacy will substitute for one year of the required experience.


A doctorate in pharmacy will substitute for the two years of required experience.


Completion of an accredited pharmacy residency program will substitute for the two years of required experience.

Examples of Work


Dispense drugs to wards, clinics, and clients using proper pharmacy procedure.


Assure the monitoring and evaluation, with medical staff input, of the quality and appropriateness of patient services provided by the pharmaceutical department or service.


Prepare IV piggybacks, large volume parental, chemotherapy solutions, and parental nutrition hyperalimentation solutions.


Participate in patient care rounds to provide drug information regarding therapeutics and medical problems or questions.


Assist in instructing and training staff Pharmacists, residents, and medical and other health care students in therapeutics and clinical pharmacy functions.


Participate in aspects of hospital quality assurance program relating to drug utilization and effectiveness.


Provide drug monitoring services which may include monitoring pharmacokinetic drug levels.


Instruct patient or appropriate nursing staff personnel who advise patient, verbally or in writing, on importance, correct use, and possible side effects of medication to be taken following discharge, in accordance with physician's instructions or written medical staff policy.


Reviews patient's drug regimen for potential interaction, interferences, or incompatibilities prior to dispensing drugs to patient, resolves irregularities with prescribing physician, and notifies nursing service and hospital administration when appropriate.


Collect information in the pharmaceutical department/service, or through hospital quality assurance program, concerning aspects of pharmaceutical services.


Assess data to identify problems in patient care and opportunities to improve care.


Determine usage patterns for drugs according to clinical department or individual prescribers and assists in setting drug use criteria.


Communicate new product information to nursing service and other hospital personnel, as required.


Participate in meetings of the pharmacy and therapeutics committee and implements decisions of that committee.
