Responsibilities

Positiion Summary

The Orthopedic Coordinator - RN is responsible for the development coordination and maintenance of the care continuum for patients undergoing orthopedic surgeries.

The Orthopedic Coordinator-RN provides leadership for all aspects of the continuum of care beginning before surgery through 90 days post operatively. This includes: pre-admission testing perioperative services inpatient unit and all other applicable entities within the orthopedic service line related to evidence-based practice for orthopedic patients.

Assists in the planning development and implementation of program goals and operations.

Coordinates and oversees the ongoing disease-specific certification process to continually improve care for the orthopedic patient population.

Works collaboratively with physicians physician office staff care management home care programs and rehabilitation facility staff to promote a successful program and patient experience for orthopedic patients.

Collaborates with multidisciplinary groups at the hospital to develop and provide patient education and coordinate care on an ongoing basis.

Assists the patient and family in the smooth navigation of their needs throughout the orthopedic care journey.

Oversees the quality metrics for the program and works collaboratively with physician offices preoperative testing and admission perioperative and inpatient care areas to develop pathways/plans to address issues. Knowledgeable on orthopedic certification requirements and responsible for initial orthopedic certification and ongoing orthopedic recertification.
