What you will do…
The Case Manager is responsible for building and fostering professional relationships with individuals, family members, and members of the team to maximize the individual’s human dignity, community integration and opportunities for self-direction. This is considered a "Float" Position and will consist of roughly half the time working on Case Management and the other half will be spent working as a Skills Instructor, teaching adaptive daily living skills to individuals with developmental disabilities.
- The rate of pay is $20.00 per hour.
- This is a full time 40 hour position, Monday through Friday from 8:00am to 4:00pm

You can make a difference by…
- Monitoring follow-through and effectiveness of support plans with department staff and Interdisciplinary team in cooperation of the department manager. Providing guidance to department staff in completing required documentation.
- Planning for and participating in planning meetings (team meetings, DHSP, ISP) actively advocating for Individuals’ needs, and contributing to overall goals of the department and individuals supported.
- Coordinating or completing assessments/evaluations in order to determine support levels and support planning. Conducting re-evaluations on an as needed basis.
- Providing personal care and assistance to individuals consistent with both their physical needs and defined support plans. Providing coverage to groups as needed.
Qualifications and Skills
- High School diploma is required, a Bachelor's Degree in Human Services or a related field is preferred.
- At least one year of experience supporting adults with developmental disabilities is preferred, a passion and dedication to supporting our people is a must.
- A valid driver's license, acceptable driving record and reliable transportation is required.
- Successful candidates will be required to complete a background check including references, CORI and fingerprinting.
