HUD Case Manager

Purpose
To provide housing case management to HUD assisted clients seeking housing and services in the New Haven region. This position will primarily support clients in the housing application process, housing search, and independent living with the ultimate goal of self-sufficiency and housing stabilization.
Duties and Responsibilities
- Assess needs and strengths to develop client’s goals.
- Assist with identification and documentation for securing Basic Needs and entitlements such as Medicaid, Supplemental SS, SSDI, Nutrition Assistance, housing subsidies etc.
- Help identify, secure and occupy safe & affordable housing.
- Assist with budgeting, bill-paying, housekeeping, shopping and cooking.
- Help integrate mental health and addiction treatment and use of medical and wellness services.
- Encourage social supports such as peer activities and groups; develop connections to the community.
- Connect tenant to job services and employment alternatives such as volunteer work or steps towards achieving employability such as a driver’s license or completing vocational training.
- Attend weekly and monthly Columbus House, program and network meetings and outside provider meetings as designated Other related duties as assigned.
Qualifications, Knowledge, Skills and Abilities
- Bachelor’s Degree or Associates with 3 years’ experience in the human services field.
- Experience with case management, supportive housing or a related field preferred.
- Individual must be able to work both independently and within a team setting.
- Knowledge of Fair Housing laws, mental health, substance abuse, and homelessness; sound judgment and decision making.
- Must have solid math and time management skills.
- Group facilitation experience a plus.
- A strong candidate is highly organized and attentive to detail.
- Motivated to complete tasks and see consumers succeed is also important.
- Must possess excellent writing skills.
- Must have a valid Driver’s License with a good driving record; computer literate.
- Bi-lingual English/Spanish preferred.
Supervisor: Permanent Supportive Housing Manager
Salary Range: $19.23-$21.00 depending on education and experience
Schedule: Monday – Friday 8:00 a.m. to 4:00 p.m.
